The most common problem for the account creation failing is the attempted use of a password that does not meet the MS complexity requirements.
You must use 3 of 4 set of characters in a string at least 14 characters long.
1) Lower case Letters
2) Upper case Letters
3) Numbers
4) Special characters ! @ # $ _ % ^ & *
