Prior to a move
As soon as a move is contemplated, submit this move ticket. Please have the following information ready to complete the ticket:
- Name of the person to be moved
- Date of Move
- Current location
- New location
- Current phone jack location (Building, room number and the number on the wall outlet)
- New phone jack location (Building, room number and the number on the wall outlet)
You will still need to notify:
- Telecom by placing a Verasmart ticket at https://app.calero.com/cwu/VeraSMART
- Lock Shop (x3000) regarding keys needed for the new location
- Surplus & Asset Management (x2157) to remove any surplus items prior to the move
- Custodial transportation services (x2984) to perform a "pre-move walk-through" to plan for furniture relocation.
- Facilities by placing a ticket at https://fmdapps.cwu.edu/fmdrs/login
This ticket will notify the following departments:
- Networks and Operations (who will need to know early in the move if the location will requires the addition of new jacks, re-cabling or if the move is to be scheduled during break or holiday periods.)
- Computer Support Services (will move computers)
